We would appreciate if Guild members who have donated items to the Guild would complete this form (even if you do not want a tax donation letter from the Guild) to help us more accurately report our degree of "public support" .  The info will be kept confidential.  Phyllis will have a yellow folder at the meetings, into which you may put your form.
